Whether your company is
purchasing a membership plan or looking to offer the best membership rate for
the employees to pay themselves, we will work closely with you to develop a plan

Payroll Deduct Plan
To establish this program, your company contact will arrange a sign up period
with the Gold’s Gym Corporate Account Manager. The employee will sign an
agreement with Gold’s Gym and also with the company payroll department. The
company will receive a monthly invoice and will deduct the initiation fees and
monthly dues through monthly payroll deductions. Employees can be added to this
plan at any time and can be removed if they leave the company. Adding and
removing employees is handled over fax or e-mail.

Company Purchase Plan
The plan allows the employees to enroll with no out-of-pocket expenses. Your
company contact will arrange for an on-site sign up with the Gold’s Gym
Corporate Account Manager. At the sign up, Gold’s Gym will enroll all interested
employees and invoice the company for the total number of memberships. From this
point, your company can either absorb the full cost of the membership or deduct
a portion of the membership fee from the employee’s paycheck. If the company
absorbs the cost, the memberships will be owned by the company. If the employee
leaves the company for any reason, the departing employee can be removed from
the slot and another employee can use the remaining time.

Provide a company contact person to handle any
questions that Gold’s Gym may have concerning the program. |

Inform Gold’s Gym of any changes in your company that
may affect membership. (e.g. Hiring of more associates, terminating
associates, etc.) |

Give each associate that would like to participate in
the Gold’s Gym Corporate Fitness Program a temporary Corporate Membership
Card. The associates will need to take the temporary card and a picture I.D.
(driver’s license) to the Gold’s Gym facility in order to obtain their
permanent membership number. |
